Referral and Acceptance of Filipino Candidates Entering a

Training Facility for Kaigofukushishi

under the Chapter on Movement of Natural Persons (MNP) Article 110.1 (f).of the Japan-Philippines Economic Partnership Agreement (JPEPA)


Qualification Requirements

  1. 21-30 years old

  2. Graduate of any four year health related program

  3. Have a general weighted average of 2.5 or its equivalent

  4. Physically, psychologically, mentally and emotionally fit

  5. Have no pending administrative or criminal case and must not have been convicted of any crime

  6. Be able to satisfactorily show financial support for his/her study and stay in Japan

  7. Does not fall under any of the following conditions:

  • conditions for refusal of entry under Article 5 of the Japanese-Control and Refugee-Recognition Act

  • belong to any form of military service

  • pregnant


Documentary Requirements

  1. A letter of application addressed to the CHED Chairman

  2. Accomplished application form

  3. Original and certified photocopy of birth certificate (NSO)

  4. Original and certified photocopy of Transcript of Academic Records (Baccalaureate/Graduate)

  5. Original and certified photocopy of Diploma (Baccalaureate/Graduate)

  6. Latest NBI clearance (valid for the last 6 months)

  7. Medical certificate from CHED accredited hospitals (for short-listed applicants only)

  8. Result of Psychological Test (for short-listed applicants only)

  9. Updated bio-data/resume

(Spell out acronyms of organizer/s and topic/s or subject/s of the training

programs)

  1. Letter of reference from school or most recent employer

  2. For exemption from 6 months Japanese Language Training

  • Certificate of the Japanese Language Proficiency Test (Level 1 or 2) or Certificate of Learning for 6 months or more at a Japanese Language School designated by the Japanese Ministry of Justice

  1. Proof of financial capability

  1. Individual Sponsor

a.1. Affidavit of support subscribed and attested before the Philippine Consulate/Embassy nearest to the residence of the sponsor

a.2. Proof of support

  • bank statement

  • real property assets

  • employment certificate

  • relationship to applicant


b. Institutional Sponsor

b.1 Affidavit of support duly approved by the Board of Directors of the said institution

b.2. Proof of Support

  • financial statement

  • certificate of good standing by government regulatory agency that has jurisdiction over the said institution

  • sponsorship contract

b.3. Certificate of sponsorship from JICWELS-accredited training institution


Conditions For Filipino Applicants

  1. Shoulder the tuition cost in the training facility for Kaigofukushishi and necessary living expenses to study and stay in Japan

  2. Shoulder cost for application forms and other related expenses such as photos, acquisition of a medical check sheet and other certificates, domestic postage to send the application forms to the CHED, domestic travel expenses to participate in the pre-departure orientation/seminars, and domestic travel expenses to the international airport at the time of departure

  3. Not allowed to work during six months Japanese Language Training (Japanese Language training is free of charge including board and lodging)

  4. Must satisfactorily meet the scholastic requirements and institutional conditions required in the study of the Kaigofukushishi course

  5. Satisfactorily meet such other conditions stated under the MOU between CHED and JICWELS and Information Guide to MOU between CHED and JICWELS
